2004 Community Health Assessment
The Community Health Assessment provides a
detailed look at health data surrounding a number
of broad public health issues. Topics include demographics, socio-economics, maternal
and child health, youth issues, chronic diseases, communicable
diseases, diversity issues, senior issues, social issues and
health care services.
Our hope is that this document will act as a catalyst to determine areas of priority and
will provide opportunities for
dialogue and collaboration among community leaders, planners
and organizations.
This assessment will also act as a resource for relevant community health data. During
the summer and early fall of 2005, community meetings with various
organizations will be conducted to initiate strategic planning
as issues are discussed and health problems that have the greatest
priority are identified.

The 2004 Community Health Assessment was completed
by assembling and analyzing data which is routinely collected
by many local, state and federal agencies. An original health and risk factor survey of county residents
was also completed. This information is vital to understanding what
is presently occurring among Greene County residents. The survey will also provide
valuable information that is not
routinely collected and distributed.
This assessment was completed in 2004 and was made possible through grant funding provided by the Missouri
Foundation for Health.

Learn More . . .
The assessment is an extension of the 1996 health assessment project
which provided the foundation for the strategic planning
for many programs and initiatives in the community. It also resulted in the
routine distribution of health indicator
data to the community through annual community health and environmental
health report cards, as well as a detailed Environmental Health Assessment
completed in 2002.
