  1. Employees aer not always washing hands when necessary when: person taking phone orders with gloves on then handled ready-to-eat chips w/o washing hands, employee handled dirty dishes with gloves on and then handled "clean" dishes with same gloves.
  2. bare hand contact observed with ready-to-eat food when: bartender cuts strawberries for drinks w/o gloves, cook rinsed lettuce for sandwiches w/o gloves. (lettuce discarded)
  3. Quat sanitizer being dispensed at 400ppm, code does not allow >200ppm for restaurant use.
  4. Unclean equipment: can opener blade, knives in rack, vegetable chopper, tomato slicer. Removed for cleaning.
