What to bring for your first WIC visit


In order to enroll you and your family in WIC, we must complete state and federal program information certification paperwork. To help us make that process as smooth as possible for you, please bring with you as much of the following information as possible. If you do not have some of the information on this list, please come to your appointment anyway! Please allow two hours for your first appointment. If you are attempting to certify more than one person, allow additional time beyond the two hours.
