Program Overview . . .
The health department maintains an aggressive program designed to identify cases of sexually transmitted disease, assure that all cases have been treated correctly, educate the public on risk factors and prevention methods and break the chain of infection by locating and notifying contacts of known infections.

Case Management . . .
The HIV/AIDS Ryan White Case Management program provides assistance in locating, coordinating and authorizing medical and psychosocial services for individuals with HIV/AIDS.

The health department works closely with AIDS Project of the Ozarks to provide case management services to a 29-county area in southwest Missouri.

The program is provided at no cost to clients who are HIV positive and reside in Missouri. Eligibility is not based on financial or insurance status.

Clients work with case managers in strict confidentiality to develop a plan based on individual need, eligibility and available funding. Case managers work to provide access to appropriate services such as medical care, housing, counseling, transportation, medication, dental care, food, home health, education, support groups and insurance continuation.

With the client's permission, referrals into the program can be made by providers, family, friends, hospitals and community agencies by calling (417) 864-2044 or (417) 864-0255

I have been a part of the Springfield-Greene County Health Department's HIV/AIDS program since May of 1999.

I was diagnosed and receive my case management from the health department.

I have been most pleased and always continually impressed with their level of professionalism and courtesy.

In a time in my life when I felt the most vulnerable, I was treated with the utmost dignity and respect.

I wish to say thank you to all of the health department staff and services for all of their continued good work and services.

- Michael
