Walk-In Accident Reports

Walk-In Accident Reports are designed for reporting vehicle accidents in person when there has only been damage to the vehicles involved in the crash, or to report damage caused by an unknown driver who leaves the scene of the accident. The police department cannot work accidents that happen on private property, such as a mall parking lot, unless there are injuries that require immediate medical attention, or drinking is involved. Criteria that an accident must meet to be filed on a Walk-In Accident Report:

  • Accident happened within the Springfield city limits
  • Property damage to vehicles only
  • No injuries requiring immediate medical attention
  • There was no drinking involved
  • All vehicles involved are operational

Each driver should exchange the following information:

  • Name and date of birth
  • Address
  • Home and work phone numbers
  • Driver's license number and state of issue
  • Vehicle information (make, model, year, color, license plate number and state of issue, etc.)
  • Insurance information

You may go to the Front Desk at Headquarters between 7:00 a.m. and 11:00 p.m. seven days a week and complete a Walk-In Accident Report. If you prefer, you can pick up a form at the Front Desk or print one to complete. Return the completed form to the Front Desk or mail it to:
