Services are delivered in the home during scheduled visits. We answer questions, monitor blood pressure and weight, connect families with community resources and teach ways to have a healthier pregnancy and a healthier baby.

During prenatal home visits, our nurses will follow the pregnancy progress and provide follow-up visits after the baby is delivered


During one-on-one sessions, our nurses help you learn more about infant health, safety and development as you love and nurture your new baby.


During infant care service, meet with one of our nurses to discuss your questions and concerns. It's like having a nurse in the family.


We provide educational classes on a variety of topics for anyone involved in caring for the baby. These classes help your family stay well, safe and happy.

Who qualifies for NEST Partnership services?

Prenatal qualifiers:

  • In Greene County
  • Must be pregnant
  • Medicaid recipient or in the process of signing up
  • Have 2-3 medical risk factos
  • Agree to a nurse contacting them about services

Infant qualifiers:

  • In Greene County
  • Currently not receiving home health care
  • Medically fragile infant with a history of NICU or other medical condition
  • Limited social support
  • Weight gain issues
