The Community Health Worker Initiative (CHWi) works to ensure that all Greene County residents have access to quality, culturally competent services.

The Community Health Worker Initiative (CHWi) is made possible through a grant from the Missouri Department of Health and Senior Services with funding from the Centers for Disease Control and Prevention. This program enables local public health agencies, like Springfield-Greene County Health, to control risk factors for severe illness caused by chronic diseases and COVID-19. 

The CHWi program works with people who are at the highest risk of poor health outcomes and those with chronic health conditions like obesity, diabetes, hypertension, heart disease, and chronic kidney disease. The goal of the CHWi program is to protect and improve community health and the conditions where people are born, live, learn, work, and worship.. The CHWi team of Community Health Advocates are matched with individuals in our community to provide education and referrals to healthcare and community resources to improve their health and wellbeing.  In collaboration with community partners, the CHWi program works to connect with residents in Springfield and Greene County.

Where can I find the CHWi program? 

The Community Health Advocates set up to meet with clients at partner locations around Greene County. If you would like to meet an Advocate in person to discuss the ways the CHWi program can assist you, you can find them at any of the Health Department's COVID-19 Vaccine Clinics or one of the events below.
