Walk-ins only. To accommodate demand for the new booster offerings this clinic will be Moderna ONLY. All minors must be accompanied by a parent or guardian. Primary and Booster doses available for 6+. Booster information can be found at www.vaccine417.com . All individuals receiving a primary (1st or 2nd) dose or booster receive a $50 gift card. All individuals who are eligible to receive a gift card are required to show a photo ID before receiving their gift card. No vaccines given to ages 6 months-4 years
