Emergency Communications Compliment or Complaint

  1. This complaint form is used for the documentation and logging of complaints against the department or personnel. Any complaint logged in this form will receive appropriate investigations according to Standard Operating Guideline 133. Per SOG 133, investigations will be given a deadline of 45 days for completion. Any need for extensions beyond 45 days will be communicated to the complainant. Furthermore, an investigation status will be provided to the complainant if the investigation exceeds 30 days.
